  • Swasthya Mitra: Field representatives appointed by the Company for data collection, patient guidance, and awareness activities.

  • Services: Includes patient referral, hospital facilitation, awareness programs, Ayushman Bharat scheme support, emergency transport, and healthcare consultancy.

 

 


 

 

2. Scope of Services

 

 

The Company provides the following services:

 

  • Verification and accreditation of hospitals.

  • Facilitating patients under government schemes such as Ayushman Bharat.

  • Organizing health camps and awareness drives.

  • Swasthya Mitra deployment for rural health data collection and assistance.

  • Emergency patient transport services through ambulances.

  • Maintaining a verified list of hospitals for public access and referrals.

 

 

Please note:

The Company is not a hospital itself and does not provide direct medical treatment. Treatment and care are provided by the Hospital Partners independently.

 


 

 

3. Eligibility

 

 

You must be at least 18 years old to use our services independently. If you are under 18, you may use our services only with the involvement and consent of a parent or guardian.

 

For hospitals and other institutional users, legal authorization and compliance with regulatory norms are mandatory.

 


 

 

4. User Responsibilities

 

 

  • Provide accurate, current, and complete information during registration or service usage.

  • Use the services only for lawful purposes.

  • Maintain confidentiality of account information, login credentials, and associated data.

  • Immediately notify us in case of any unauthorized access or security breaches.

 

 


 

 

5. Hospital Partner Responsibilities

 

 

  • Maintain the quality standards agreed during the accreditation process.

  • Provide safe, ethical, and timely healthcare to referred patients.

  • Keep the Company informed of any changes in facilities, certifications, or services.

  • Abide by all applicable healthcare regulations, medical ethics, and patient rights obligations.

 

 

Failure to comply may result in suspension or termination from our Accredited Hospital List.

 


 

 

6. Payment Terms

 

 

  • For some services (e.g., Ayushman Bharat facilitation, emergency transport, special referrals), nominal service fees may apply, payable as per agreed terms.

  • All applicable taxes and charges must be paid by the user or hospital as per statutory requirements.

  • Late payments by hospitals or institutional clients may attract penalties or suspension of services.

 

 


 

 

7. Limitation of Liability

 

 

While we strive for excellence, the Company shall not be held liable for:

 

  • Any medical negligence, malpractice, or treatment outcomes at partner hospitals.

  • Any errors, delays, or omissions in information shared by hospitals.

  • Any acts beyond our reasonable control including emergencies, strikes, cyber-attacks, or natural disasters.

 

 

Our liability in any case shall be limited to the fees paid (if any) for the specific service involved.

 


 

 

8. No Medical Advice

 

 

Information provided through our website, Swasthya Mitras, awareness programs, or any other channel is for informational purposes only.

It does not constitute professional medical advice, diagnosis, or treatment recommendations.

 

Always consult a qualified doctor or hospital before making any healthcare decisions.

  • Efforts will first be made to resolve through mutual negotiation.

  • If unresolved, the matter will be referred to arbitration under the Arbitration and Conciliation Act, 1996.

  • The seat of arbitration shall be Seoni, Madhya Pradesh, and the language of arbitration shall be English.

These Terms shall be governed and construed in accordance with the laws of India.

Any legal proceedings shall be subject to the exclusive jurisdiction of courts in Seoni, Madhya Pradesh.
